Apple introduced Sensitive Content Warnings in iOS 17 and iPadOS 17 to prevent unsolicited nude photos and similar content from coming through to your iPhone or iPad. This feature does not work by default, it has to be enabled.

Enabling Sensitive Content Warnings in iOS 17 ensures that incoming files, videos, and images undergo on-device scanning. If the content contains nudity, it is automatically blocked. You can choose to opt-in for blurring, which can be applied to images across various platforms, including Messages, AirDrop, Contact Posters in the Phone app, FaceTime messages, and third-party applications. The Messages app will automatically blur the sensitive content and require user consent to view the material.
